Kosmic: The Bleeding Wheel

Every cosmology says the planes are elsewhere, reached through careful doors. In Kosmic the Wheel turns too close. The planes grind against the mortal world, and where they press hardest, reality tears. Through those tears they don't send armies; they send themselves. A valley wakes as the Forge. A city's color drains. The world is not being invaded. It is being overwritten, one wound at a time, and Hearth, the last warm, ordinary place, is where you stand.
